By submitting this form, you acknowledge and consent to the collection and processing of your personal information by the Covary Research Program (CRP) and it's parent ChordexBio. The information you provide will be used exclusively for the purpose of administering, evaluating, and processing your application to the program. Your data will be handled confidentially and will not be shared with third parties outside ChordexBio and the CRP administrative and mentoring team, unless required by law or with your explicit permission. We store your information securely, and access is limited only to personnel directly involved in program management, mentorship assignment, applicant communications, and program improvement. You understand that: (1) your participation is voluntary; (2) you may contact us at any time for questions regarding your data, to request corrections, or to withdraw your information from our records (except when the data is already necessary for an active program involvement).
